What do I need to bring to my appointments?

Please bring the following to your appointment with you: photo identification, insurance card(s), a list of all medications you are currently taking and dosages, any previous MRI, x-rays or other imaging studies that you may have in regards to your reason for seeing the doctor and referral information.
How do I get a copy of my medical records?

Due to patient privacy laws, all requests for medical records must be made in writing.
Will I have to pay for parking?

Parking is free at all Roper St. Francis Healthcare facilities including both of our office locations. (some may require for you to get your parking validated by the front desk staff)
